Why Hospital Discharge Is a Critical Moment

A hospital stay — whether for a fall, infection, surgery, or stroke — takes a serious toll on the body. Older adults can lose up to 5% of their muscle strength for every day spent in bed. After a week on a ward, your parent may struggle to manage stairs, get in and out of bed, or walk safely to the bathroom. The medical problem may have been treated, but the deconditioning is a problem of its own.

NHS community therapy waiting lists in Hampshire and Dorset can stretch to several weeks or longer. During this gap, many patients lose confidence, become more dependent, or have another fall — sometimes leading to readmission. Research shows that early rehabilitation after discharge is one of the most effective ways to break this cycle.

If the hospital says your parent is "medically fit for discharge" but you're worried they're not ready to cope at home, you're right to be concerned. A rapid-response home assessment by a physiotherapist or occupational therapist can identify what support is needed and start rehabilitation before things deteriorate further.

How Our Team Helps After Hospital Discharge

Our physiotherapists and occupational therapists work together to reverse the physical deconditioning from a hospital stay and ensure your parent can manage daily life safely at home.

Physiotherapy

Rebuilding lost muscle, improving walking safety, and restoring the confidence to move around the home independently.

  • Deconditioning Reversal — Targeted strengthening exercises to rebuild muscle lost during bed rest.
  • Gait Re-education — Correcting shuffling, unsteadiness, or walking aid dependence that develops during hospital admission.
  • Balance & Falls Prevention — Rebuilding balance reactions to reduce fall risk during the vulnerable first weeks at home.
  • Stair Practice — Safe, supervised practice on your parent's own staircase with their specific handrails and carpet.
  • Outdoor Mobility — Progressing to walking outside, practising uneven surfaces and kerbs to rebuild confidence.

Occupational Therapy

Ensuring your parent can manage daily life safely after leaving hospital, from washing and dressing to making a cup of tea.

  • Home Safety Assessment — Thorough check of trip hazards, bathroom safety, bedroom access, and lighting before problems arise.
  • Equipment & Adaptations — Advising on grab rails, shower stools, bed levers, and other aids while avoiding unnecessary purchases.
  • Daily Living Skills — Practising washing, dressing, and cooking using techniques that work with your parent's current ability.
  • Fatigue Management — Teaching pacing and energy conservation strategies to prevent setbacks from overdoing it.
  • Cognitive & Delirium Support — Adapting approaches for post-hospital confusion and coordinating with our dementia support service if needed.

Why Rehabilitation at Home Works Best After Discharge

You cannot learn to live in your own home by practising in a hospital gym. Successful rehabilitation happens when exercises are meaningful and relevant — carried out in the environment where your parent actually needs to function.

By treating patients at home, we practise on your parent's stairs, work with their furniture, and identify hazards in their home before they cause a fall. We see the bathroom they need to get in and out of, the kitchen they need to make a meal in, and the front door they need to manage independently.

This "real-world" approach builds confidence faster and ensures that every recommendation is practical and specific. Families are involved from the start, so you know exactly how to support your parent's recovery between sessions.

Frequently Asked Questions

How quickly can you start after hospital discharge?

We can often arrange a first home visit within 48 hours of your call. If discharge is imminent, contact us as soon as possible — we can help prepare the home and have a therapist ready to visit shortly after your parent arrives home. Early intervention produces the best outcomes.

Do you work alongside NHS community teams?

Yes. Many families use us to supplement NHS therapy when community waiting lists are long. We coordinate with NHS therapists, GPs, and hospital discharge teams, sharing progress notes with your consent. We step in where NHS provision has gaps — and step back when NHS services come on stream.

My parent is confused after their hospital stay. Can you still help?

Post-hospital confusion (delirium) is very common in older adults, particularly after anaesthesia, infection, or a prolonged stay. Our therapists are experienced in adapting their approach for patients with confusion, and can coordinate with our dementia support service if ongoing cognitive difficulties are identified.

Can you help before discharge, while we're still planning?

Absolutely. In fact, planning ahead produces the smoothest transitions. Call us before discharge and we can advise on preparing the home, recommend any equipment that should be in place, and arrange for a therapist to visit soon after your parent arrives home. This reduces stress for everyone.

Which areas do you cover for hospital discharge rehabilitation?

We provide home visits throughout Hampshire, Dorset, and Wiltshire — including patients discharged from Southampton General, Royal Bournemouth, Poole Hospital, Salisbury District Hospital, and Lymington New Forest Hospital. Our areas include Southampton, Bournemouth, Poole, Salisbury, Ringwood, the New Forest, and surrounding towns and villages. View all areas we cover.
